Each of our new patients will need to complete our initial informational form through this site. We also need our Policies & Procedures Agreement signed and brought to the first meeting. It’s also important to actually read our Policies & Procedures Agreement. It’s nothing earth shattering, but gives you a good idea what to expect.
Once a patient or parent has made contact with us through the website we’ll give you a login to the introductory form. This form is very detailed but best serves our patients and us. Giving such thorough information before our first visit means we’ll already have a good idea of what your needs are. You may have told your story many times; this form keeps you from needing to tell it from the beginning again.
